COVID-19: The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. It is believed that an individual can be in infected with COVID-19 without their knowledge and be asymptomatic. Warren Academy and its partners have put in place preventative measures to reduce the spread of COVID-19. However, Warren Academy and its partners cannot guarantee that I or anyone else will not become infected with COVID-19, including my family, guests, or friends. Participation in a Warren Academy athletic sports program(s), related event, or activity, could increase the risk of contracting COVID-19. By signing this agreement, I ACKOWLEDGE the contagious nature of COVID-19 and VOLUNTARILY ASSUME THE RISK that I may be exposed to or infected by COVID-19 by participating in a Warren Academy athletic sports program(s), related event, or activity, and that such exposure or infection may result in personal injury, illness, permanent disability, and death to family member, guests, friend, or associate. I understand that the risk of becoming exposed to or infected by COVID-19 at a Warren Academy program(s), related event, or activity may result from the actions, omissions, or negligence of myself or others, including, but not limited to, Warren Academy employees, partners, volunteers, and program participants. I UNDERSTAND AND VOLUNTARILY ACCEPT AND ASSUME ALL the foregoing risks related to COVID-19 and accept sole responsibility for any injury or illness that may occur. Further, I UNDERSTAND AND AGREE that this release includes any Claims based on the actions, omissions, or negligence of Warren Academy, its employees, partners, agents, volunteers and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Warren Academy athletic sports program(s), related event, or activity.

ATHLETE WAIVER/RELEASE FORM (“AGREEMENT”) In consideration of being permitted to participate in any way. I, for myself, my personal representatives, assigns, heirs, and next of kin: 1. ACKNOWLEDGE, agree and represent that I understand the nature of the Warren Academy event and that I am qualified, in good health, and in proper physical condition to participate in such. I further agree and warrant that if at any time I believe conditions to be unsafe, I will immediately discontinue further participation. 2. I FULLY UNDERSTAND that: (a) ATHLETIC ACTIVITIES INVOLVE RISKS AND DANGERS OF SERIOUS INJURY, INCLUDING PERMANENT DISABILITY, PARALYSIS, AND DEATH (“RISKS”); (b) these Risks and dangers may be caused by my own actions or inaction, the actions or inaction of others participating in the Warren Academy event, the condition in which the Warren Academy event takes place, or THE NEGLIGENCE OF THE ‘RELEASES’ NAMED BELOW; (c) there may be OTHER RISKS AND SOCIAL AND ECONOMIC LOSSES either not known to me or not readily foreseeable at this time, and I FULLY ACCEPT AND ASSUME ALL SUCH RISKS AND ALL RESPONSIBILITY FOR LOSSES, COSTS, AND DAMAGES I incur as a result of my participation or that of the minor in the Warren Academy event. 3. I HEREBY RELEASE, DISCHARGE, AND COVENANT NOT TO SUE Warren Academy (WA), including its parent company, related affiliated and subsidiary companies of each, as well as the officers, directors, agents, employees and assigns of each, and the Warren Academy Associations, clubs, coaches, officials, administrators, members, volunteers, participants, sponsors, advertisers, and if applicable owners and lessors of premises on which the Warren Academy Training takes place, and any other party indemnified and held harmless by the WARREN ACADEMY (each considered one of the ‘RELEASES” herein) FROM ALL LIABILITY, CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON MY ACCOUNT CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PARTY BY THE NEGLIGENCE OF THE “RELEASES” OR OTHERWISE, INCLUDING NEGLIGENT RESCUE OPERATIONS, NEGLIGENT SECURITY, TRAVEL, AND RECREATIONAL OPERATIONS AND ACTIVITIES; AND I FURTHER AGREE that if, despite this RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT I, or anyone on my behalf, makes a claim against any Release, I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS BEACH OF THE RELEASES from any litigation expenses, attorney fees, loss, liability, damage, or cost which may incur as the result of such claim.

I consent to have medical treatment that may be deemed advisable in the event of injury, accident and/or illness during any program, event or activity. I release Warren Academy and its partners and all persons participating in any such medical treatment from all responsibility for any such actions.

I understand that Warren Academy and its partners will be filming and taking photos at this event. I also understand that Warren Academy and its partners may use my photograph, recordings or other record of me at the event for any legitimate purpose.

If you wish to cancel, we must receive your request in writing 24 hours prior to the event starting. Any initial fee is non-refundable for any reason other than medical. MEDICAL REFUNDS WILL NOT BE ALLOWED AFTER THE EVENT, – NO EXCEPTIONS! Requests for medical refunds, must be in writing accompanied by a physician's letter of explanation prior to event.
