Athlete Waiver and Liability Release
ATHLETE WAIVER/RELEASE FORM (“AGREEMENT”)In consideration of said athlete being permitted to participate in the FROM THE THE FIELD TO THE COURT YOUTH FOOTBALL AND BASKETBALLL CAMP, which will be referred to as THE CAMP, the personal representatives and or the legal guardian: 1. ACKNOWLEDGES, agrees and represents that I understand the nature of the THE CAMP and that the athlete participating is qualified, in good health, and in proper physical condition to participate in THE CAMP. 2. I FULLY UNDERSTAND that: (a) ATHLETIC ACTIVITIES INVOLVE RISKS AND DANGERS OF SERIOUS INJURY,(“RISKS”); (b) these Risks and dangers may be caused by the participating athlete’s own actions or inactions, or the actions or inactions of others participating in the THE CAMP. 3. I HEREBY RELEASE, DISCHARGE, AND COVENANT NOT TO SUE Warren Academy (WA), The Tolliver Family Foundation, DREAM, related affiliated and subsidiary companies of each, as well as the officers, directors, agents, employees and assigns of each, clubs, coaches, officials, administrators, members, volunteers, participants, sponsors, advertisers, and if applicable owners and lessors of premises on which the THE CAMP takes place. (Each considered one of the ‘RELEASES” herein) FROM ALL LIABILITY, CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON MY ACCOUNT CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PARTY BY THE NEGLIGENCE OF THE “RELEASES” OR OTHERWISE. INCLUDING NEGLIGENT RESCUE OPERATIONS, NEGLIGENT SECURITY, TRAVEL, AND RECREATIONAL OPERATIONS AND ACTIVITIES; AND I FURTHER AGREE that if, despite this RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT I, or anyone on my behalf, makes a claim against any Release, I WILL INDEMNIFY, AND HOLD HARMLESS BREACH OF THE RELEASES from any litigation expenses, attorney fees, loss, liability, damage, or cost which may incur as the result of such claim.
Image Release
I authorize Warren Academy, Tolliver Family Foundation, DREAM and their partnering organizations permission to use my and/or my minor children’s likeness in a photograph and/or video in any and all publications and materials (print and electronic) without payment or consideration made. I realize these photos and/or videos become the property of said organizations and their partners. I waive the right to review any photo or video or to obtain royalties from the photo or video.
